Office Hours and Appointments
Our office is open from 8:00 a.m. until 5:00 p.m. Monday through Friday. We do not close for lunch hours but we do close for the traditional holidays. Weekend appointments are provided for cycle monitoring and special tests.
Our office hours are by appointment only so we can provide the most convenient and best service for you.

In consideration for other patients undergoing treatment, please do not bring babies or children to your appointments.
New Patient Forms
Your very first appointment with Fertility Physicians is a “New Patient Consultation.” When you make your appointment for a New Patient Consultation, we will mail you a packet of information and forms to review and complete before the appointment. This information will provide your FPNC physician with details that are important and relevant to your reproductive care.
In order to prepare your chart before your initial consultation appointment, please obtain or complete the information listed and mail or fax it to our office right away. If you are not able to forward the paperwork in time for it to reach us before your appointment, please bring the information with you, arriving at least 30 minutes before your appointment so we can prepare your chart.
If your appointment is only days away, you might prefer to download the forms (listed below) and fax them to FAX# 408-907-0999:
- Patient and partner registration forms
- Patient gynecologic history and partner history form
- Patient medical history form
- Copy of health insurance card(s) both sides: front and back
- Medical records (OB/GYN and other relevant records. You can request these records using this medical records release form.)

New Patient Authorization
The fee for your New Patient Consultation appointment is usually covered by insurance. If you wish, our patient services advisors can provide you specific information about your coverage before your appointment. Your patient services advisor will be your ongoing liaison for financial information related to your treatment.

PLEASE NOTE: If you are part of an HMO plan, you must obtain prior authorization in order for your New Patient Consultation to be considered for reimbursement by your insurance plan. Without prior authorization, your HMO may reduce your benefits significantly.
To obtain prior authorization follow these steps:
- Contact the primary care physician (PCP) or OB/GYN listed on your insurance and request an authorization.
- The physician will review your chart and contact the medical group to request a letter of authorization on your behalf.
- The letter is typically mailed to the patient (sometimes a fax is possible).
- Please note it can take up to 72 hours to get approval.
Preparing for Treatment
Infertility can be both physically and emotionally challenging, but there are strategies that will help you with your treatment. It is important to understand as much as possible about your treatment options and to be well informed about the procedures.
\We encourage you to write down your questions and discuss them with your nurse practitioner or your physician during your visit. We also offer educational events that provide trustworthy, accurate information and mind-body programs to help manage the stress that often accompanies infertility.
